When you visit the clinic for the first time you will be asked to complete a registration form, a copy of which is available by clicking on the link below. If you would like to complete this form prior to your visit please do so and bring it with you to your appointment.
Chaperone
Maintaining your privacy and dignity throughout your visit is of paramount importance to us.
Please be aware a chaperone will be routinely offered for any intimate examination.
Consent
We want you to feel fully informed and involved in your care and your consent will be sought for any procedure or diagnostic test.
All of our team are happy to answer any questions that you may have about your visit or ongoing care.
Hearing Loop
A hearing loop is available for use in the clinic. Please ask a member of staff if you require this during your visit.
Translation Services
Translation services can be organised on request. Please notify us of your needs at least 48 hours in advance of your appointment.
Please note the costs of any translation support required are payable by the patient and are not covered by insurance.
